Documents and Forms
Common forms and documents include: HSA contribution form, HSA transfer/rollover request form, beneficiary designation form, investment enrollment form, distribution request form, and tax documents (1099-SA, 5498-SA — available in January each year). All forms can be submitted online through your member portal or mailed to our processing center.
